What RVUs actually are

Relative Value Units (RVUs) are the foundation of medical reimbursement. They assign value to each CPT code based on the resources required to perform the service.

This system—called the Resource-Based Relative Value Scale (RBRVS)—is built on a simple idea:
the more time, skill, and cost involved, the higher the reimbursement.

The 3 components (always combined)

Every RVU is made up of three parts:

  • Work RVU (wRVU): provider time, skill, effort, and judgment
  • Practice Expense (PE RVU): overhead costs (staff, equipment, supplies)
  • Malpractice (MP RVU): liability and risk costs

These are added together, adjusted for geography, and multiplied by a conversion factor to determine payment.

How work RVUs are measured

Work RVUs capture the provider’s actual labor and are the most impactful component.

They account for:

  • Technical skill
  • Physical and mental effort
  • Clinical judgment
  • Stress related to patient outcomes

Work is also broken into three stages:

  • Pre-service: chart review, prep
  • Intra-service: the procedure/visit itself
  • Post-service: documentation, follow-up care

Time is the dominant driver across all three.

Why coding accuracy is critical

Small coding differences create large revenue gaps.

Example:

  • 99212 → 0.70 wRVUs
  • 99214 → 1.92 wRVUs

That’s a 1.22 wRVU difference per visit.

Multiply that across hundreds of encounters, and undercoding quickly turns into significant lost revenue.
